Police have identified a man who died in a Tulsa traffic collision Monday evening.
David Cacy, 44, of Tulsa was killed in a collision in the 18100 block of East Admiral Place just after 5 p.m. Monday, authorities said.
He was in a gray sedan that was eastbound on Admiral when it swerved into oncoming traffic and hit a Ford F-150 pickup head-on, reports indicate.
A toddler who was in a car seat was removed from the car by firefighters and was taken to a hospital to be checked for injuries. Two people who were in the pickup were also taken to hospitals.

This is so scary. I was almost sideswiped this afternoon when a car came into my lane to exit 169. I had to slam on my brakes to keep him from running me off the road. My thoughts and prayers are with this man's family. This is a terrible tragedy, but after coming so close today myself, I am especially thankful for one more day.

I can't believe the city isn't utilizing every possible officer to enforce traffci laws. If the state was smart, they'd outlaw cell phones while driving and rake in even more.

I was coming home tonight and young teen next to me came to a screeching halt at the red light I had stopped at in the lane next to me. She ended up about half way into the intersection before completing the stop. We could see she was on her cell phone. Heaven help us. We need this new cell phone law.

What good is a new cell phone law gonna do? It will be a fine if you get caught, but thats about it. People still drive under the influence of drugs and alcohol everyday.

I was coming out of the mall and in front of me a car weaving from one side to the other, when I passed her while she was attempting to make a turn and she nearly hit the curb, I noticed she was texting. The way she was turning had a one way sign and she was going the wrong way. Where are the cops when you need them?

I apologize in advance for saying this...but people in Oklahoma (or Texas or Georgia) cannot drive. I've lived all over the country and Tulsa has been by far the worst for driving conditions. The destraction of pot holes, pedestrians, and construction are cause for concern. I feel sad for the family and their loss. Let's use this as an opportunity to learn; drive according to the conditions, be considerate of your fellow drivers and pay attention. We can't prevent every accident but we can reduce the number of them.

I too have lived in several states and must agree with rumockingme. The drivers in TUlsa are BAD. The roads are among the worst I have ever driven on which does not help. It's not just the pot holes. The road design, lack of adequate shoulders, poor sight distance all add to the issues.
